New Patient Registration

  • Primary Care

    We provide outpatient services including all preventive and routine healthcare services, management of chronic diseases, consultation with specialists when necessary, and coordination of care across healthcare settings or multiple providers or both.

  • Behavioral Health

    We offer full psych evaluation, psychotherapy, ADHD treatment, and substance use disorder including Vivitrol injection.
